VENTRILO You dont have to have a microphone and talk but at least you will hear us talking and discussing what's happening.
Decursive is a MUST for any class that can remove debuffs.
Omen is our preferred threat meter.
Deadly Boss Mods (DBM), has built in timers and warnings for boss fights (ie. Flame Wreath for Shade 'Do Not Move' warning across center of your screen.)
There are some class specific mods as well but I dont use them and do not know all the names. I also use CT-Mod (only a few parts of it) and CT-Raid, but it looks like Blizz has incorporated alot of the CT-Raid features into the standard UI.
Please add what you use so that we get everyone on the same page.

Omni-CC is especially awesome for a DK since ability cooldowns are very complex with runes.
Death Runes occasionally hork it up, but it is totally awesome in general. =)
A few add-ons I have grown very fond of:
SmartBuff -- Lets you pre-configure how to buff in all conditions (solo, 5 man, raid) and then just does it out of combat as you scroll your mouse wheel. Perfect for any class that needs to cast buffs frequently, including Warrior shouts and DK Horn of Winter.
RatingBuster -- Shows derived stats on equipment based on your class and spec. For example, will show bonus dmg/healing from Str or AP for a Ret Paladin provided you have the proper talent. It also shows changes for gear you hover over with mouse vs. the gear you are currently wearing. The data shown is highly customizable but the default config is simply amazing out of the gate.
SexyMap -- Completely beautifies your mini-map. 110% optional, but a lot of fun.

Personally, I still recommend CT Raid for everyone, and for Balance Druids: SquawkAndAwe.

[Updated 10/09 from a previous post for Ulduar and ToC]
These are indeed the four "must haves" for the sake of raiding and group organization, to fulfill any class role: burn, soak, and crowd control (cc).
Ventrilo (Client)
http://www.ventrilo.com
Decursive
http://www.2072productions.com/?to=decursive.php
Deadly Boss Mods
http://www.deadlybossmods.com/
Omen Threat Meter
http://www.wowace.com/projects/omen-threat-meter/
Optional Mods, but musts for Healing Classes which RAID: Some kind of UI Frame which shows the health of all party members in the raid, even if it's simply setting up Blizzard's standard UI frames.
I strongly suggest X-Perl for all classes, but alternatives include CT and AG, and even Healbot
X-Perl http://www.wowace.com/projects/xperl/
CTMod http://www.ctmod.net/
For those seeking an all-in-one inventory mod, I suggest Bagnon
Bagnon http://wow.curse.com/downloads/wow-addons/details/bagnon.aspx
For those who like big Cooldown-reporting in real-time, OmniCC
OmniCC http://wow.curse.com/downloads/wow-addons/details/omni-cc.aspx
I also use Quartz, a casting-bar AddOn mainly for PvP purposes, but it is also very useful if you are an "interrupt" class.

Beefy check out Simple Raid Target Icons for easy doubleclicking marking.
Also anyone interested in a CC timer check out Afterlife Crowd control It works for 95% of cc abilities and gives you atimer for EVERY cc you have out, not just yours EVERYONE in your party/raid. some things are a little off like talented SAP it doesntgive the proper time but still for any tank I think its a must have

I have un-stickied this, as it has largely been incorporated into my new sticky post.
